Online Patient Services Latest information

Patients may use online services: online booking of appointments; online ordering of repeat prescriptions and online access to summary information within patient records (allergies, medications and adverse reactions). Please bring in your photo ID to reception and ask to register for these online services.

How to access medical records in SCL?

If you've been a patient in one of our hospitals or SCL Health Medical Group physician practices, you may request access to your medical record by downloading a copy of the Patient Request to Access Medical Records Form (also available in Spanish ), complete it and mail, email or fax it to our Centralized Release of Information Department or your local Health Information Management department to request your medical record.#N#Copies of medical records may be released to a third party (someone other than the patient) upon receipt of written authorization of the patient or guardian. To authorize the release of your records, complete the form below, then you may submit the form by email, fax or mail. Charges may apply.

Where your GP practice supports Patient Access and is able to provide it, you may additionally find a ‘Contact your GP’ form within the Health Advice feature. This added button helps you enter symptoms and then submit them to your GP for response.
