What is a PCN in Mendip?

Mendip Primary Care Network (PCN) is a collaboration between five GP practices across Mendip serving a combined population of over 38,000 patients. The network incorporates Beckington Family Practice, Park Medical Practice, Mendip Country Practice, Grove House Surgery and Oakhill Surgery. Each practice is signed-up to NHS England’s Network Contract Directed Enhanced Service (DES) Contract Specification and is required to deliver the requirements within the DES. The PCN board meets every month and each practice is represented on the board by a nominated GP and practice manager. Our clinical director is Dr Robert Weaver and PCN manager Ruth Woodland.

What is connected primary care?

Connected primary care and community teams will be the building blocks of Integrated Care Systems covering the whole country by 2021. Bringing general practices together to work at scale provides opportunities to for GP surgeries to offer a wider range of services to patients and to more easily integrate with the wider health and care system. Thirteen PCNs went live across Somerset on 1 July 2019. While patients won’t see any sudden changes, they will, over time, see a wider range of clinical services available in their local GP surgery.
