What if there is an error in my child's NCIR record?

The NCIR stores immunization records that are client-specific and created by the client's health care provider or through our data feed with NC Vital Records. If you find there is an error in your child's or client's NCIR record, please contact your health care provider or Vital Records directly.

Can My Parent access my medical records?

Due to state and federal laws providing additional privacy rights for adolescents’ medical records, patients between 13 and 17 must sign an assent agreement granting permission for a parent to access their medical record.

Pediatric interventional radiology is the technique of using minimally invasive, image-guided procedures to help diagnose and or treat a variety of illnesses and diseases.

What is the tip of a catheter?

The tip of the catheter carries a small balloon, which widens the blood vessel and improves blood flow. Sometimes a stent, a tiny mesh tube, is also inserted to support the artery walls. Feeding tube placement – Interventional radiologists place feeding tubes in children who are unable to eat by mouth.

Can a child feel pain during a syringe?

Your child will feel no pain during the procedure. Because it is important that your child stay very still during the procedure, anesthesia will be given. The anesthesia will be administered through an IV in your child’s hand, arm or foot and your child may feel a slight pinch when the IV is started.

Option 1: Sign up for MyIR to View, Download, and Print Your Family's Immunization Information

MyIR allows you to manage your family's immunization records securely online. Once you register, you can access the records any time you need them. You can also print your child's Certificate of Immunization Status (PDF) for school and child care entry. You can do all of this without an extra trip to your healthcare provider or school.

Option 2: Sign up for MyIRmobile.com

Sign up for MyIR Mobile by going to myirmobile.com and follow the registration instructions. Your registration information will be used to match your records with the state immunization registry. You will be sent a verification code on your phone which will finalize the process.

Option 3: Visit Your Local Pharmacy, Clinic, or School

Certain local pharmacies, clinics, and schools are able to register you in MyIR. If you live near one of these pharmacies, clinics, or schools, visit them today to begin the sign-up process. If you don't see a pharmacy, clinic, or school near you, try one of the other options listed on this page.

Option 4: Request a Complete Immunization Record from Your Healthcare Provider

Most healthcare providers in Washington use the Washington State Immunization Information System. Please check with your provider to ask if they can give you a complete immunization record for you or your child. They can print it from the Immunization Information System or from their own medical record system.

Option 5: Request a Complete Immunization Record from the Department of Health

If you can't get a complete immunization record from your provider and you don't want to sign up for MyIR, contact the Office of Immunization and Child Profile at 360-236-3595 or 1-866-397-0337. The Department of Health uses the Washington State Immunization Information System, but it does not have complete immunization records for all people.

Who can decide if a portal is full or limited?

Adults who grant another adult user access to their portal is able to decide if the access is full or limited.

What is a proxy access?

This allows a parent, guardian or other designated individual (like a spouse, family member, or friend) to log into their personal MyUofMHealth account, and then connect to the information regarding a family member or friend. Because of the sensitive nature of medical information, each adult must establish their own MyUofMHealth.org account, and partners cannot share access without requesting proxy access.

What is the University of Michigan's proxy process?

The University of Michigan is dedicated to protecting the information of our patients, and the proxy process was designed to meet state laws and ensure privacy. Parents of children ages 0-10 obtain full access to their child’s information.

Does University of Michigan require proxy access?

Gaining proxy access requires approval from the University of Michigan Health System, and sometimes additional information may be required to demonstrate legal authority to access health information from another patient’s portal. You will be contacted if this is the case.

What is MyChart proxy access?

MyChart proxy access allows you to log into your personal MyChart account and view your child’s information.

When does a child's proxy expire?

W hen your child turns 13: Your proxy access expires. To ensure you both have ongoing access, your child must sign the Cincinnati Children’s assent form designating at least one parent or primary caregiver as his or her proxy.

Do you have to sign a MyChart agreement with your child?

To do this, each parent signs a Cincinnati Children’s MyChart Agreement. Your child will only have to sign one Cincinnati Children’s assent form, which automatically covers both parents.

Can I have more than one child at Cincinnati Children's?

Yes. If you have more than one child who has been treated at Cincinnati Children’s, you may get proxy access to each child’s account. If you have children age 13-17, they each will need to sign a Cincinnati Children’s assent form to grant you proxy to their MyChart accounts.

Do you need a MyChart account for a 13 year old?

Patients 13 to 17. He or she must sign the Cincinnati Children’s assent form, granting at least one parent or primary caregiver proxy access before either the patient or parent can create a MyChart account. The patient is not required to have his or her own account; however, if a patient between 13-17 wishes to have an account, at least one parent or primary caregiver must have an account as well. The MyChart assent form automatically grants access to both mom and dad.

Can a parent access my chart?

No. To ensure the safety of your child’s information, patients cannot share an account with a parent and parents cannot share an account. Two parents who both wish to access their child’s information must each sign and submit a MyChart Consent Agreement and create an individual MyChart account. If the child is age 13 to 17, he or she can have a MyChart account also; you will all see the same information. See “How do I get MyChart proxy access” for more information. If your child is under 12, you can view the information in MyChart together (as appropriate) and use MyChart as an opportunity to involve your child in his or her care.

Can parents access a child's record after 18?

Click on your name in the top right-hand corner, then select Account. Scroll down to the Link a practice or user section. Select either: Link to my practice (to link to your practice). Link another patient to your account (to act on behalf of another patient as a proxy). Locate your new practice (or the patient's practice for a proxy user) ...

Can you link a proxy to a patient?

You would use this same process if you have been registered as a proxy user for a patient or relative, and already use Patient Access. This process will allow you to link your account to the patient.

Can you delete your patient account at a new practice?

You are now linked to your new practice (or the patient you are caring for) and you'll have access to all enabled services. Alternatively, for patients who have moved practices, Patient Access can send you a delete account email and you can delete your account and re-register from scratch at your new practice.
