What is a regular clinical meeting?

Regular clinical meetings are convened on a range of topics and all clinical staff are invited to attend. Individual teams also meet on a regular basis and all clinicians endeavour to have lunch together to catch up with colleagues on an informal basis.

What is a weekly meeting at the practice?

The practice holds a weekly multi-disciplinary team meeting with community colleagues to discuss high priority cases. Weekly in-house meetings are attended by all clinical staff and the practice management team, including regular significant event meetings, educational sessions, clinical audit reviews and updates on clinical pathways.

Is a patient triaged?

Patients are not clinically triaged but the reception team do ask for details of symptoms so that the most appropriate clinician can be booked. The practice has embraced technological advances and is an accredited paperless practice with all incoming clinical documents handled electronically.

The practice is rated as good for the care of people with long-term conditions. Nursing staff had lead roles in chronic disease management. Patients at risk of hospital admission were identified as a priority for appointments and health checks. Longer appointments and home visits were available when needed.

The practice is rated as good for the care of families, children and young people. There were systems in place to identify and follow up children living in disadvantaged circumstances and who were at risk, for example, children and young people who had a high number of A&E attendances.

The needs of the working age population, those recently retired and students had been identified and the practice had adjusted the services it offered to ensure these were accessible, flexible and offered continuity of care.

The practice is rated as good for the care of people experiencing poor mental health (including people with dementia). The majority of people experiencing poor mental health had received an annual physical health check.

People whose circumstances may make them vulnerable

The practice is rated as good for the care of people whose circumstances may make them vulnerable. The practice held a register of patients living in vulnerable circumstances including those with a learning disability.
